במאמר שפורסם במהלך חודש אפריל בכתב העת American Heart Journal מדווחים חוקרים על תוצאות מחקר GLORIA-AF, מחקר פרוספקטיבי עולמי להערכת טיפול ב-Dabigatran (פרדקסה) בחולים עם אבחנה חדשה של פרפור פרוזדורים ולפחות גורם סיכון אחד לאירוע מוחי.
החוקרים ביקשו לתאר את הבטיחות והיעילות של פרדקסה במהלך שנתיים בכ-3,000 חולים ממדגם GLORIA-AF עם אבחנה חדשה של פרפור פרוזדורים שאינו על-רקע מחלת מסתם, אשר היו בסיכון לאירוע מוחי.
גיוס רציף למחקר בשלב II החל לאחר אישור פרדקסה למניעת אירועים מוחיים בחולים עם פרפור פרוזדורים שאינו על-רקע מחלת מסתם. במסגרת מחקר זה בשלב II, 2,937 חולים תחת פרדקסה השלימו מעקב בן שנתיים עד מאי 2016 ונכללו בניתוח הנתונים. החוקרים התבססו על נתוני חולים שנטלו לפחות מנה אחת של הטיפול התרופתי (2,932 חולים) להערכת שיעורי ההיארעות.
החוקרים מדווחים כי שיעורי ההיארעות של אירועים מוחיים עמדו על 0.63 מקרים ל-100 שנות-אדם, 1.12 אירועי דמם מג’ורי ל-100 שנות-אדם, 0.47 מקרי אוטם לבבי ל-100 שנות-אדם ו-2.69 מקרי תמותה מכל-סיבה ל-100 שנות-אדם. בקרב חולים שנטלו פרדקסה במינון 150 מ”ג, פעמיים ביום, השיעורים המקבילים עמדו על 0.56, 1.00, 0.48 ו-2.07 מקרים ל-100 שנות-אדם, בהתאמה. בקרב אלו שנטלו פרדקסה במינון 110 מ”ג, פעמיים ביום, שיעור האירועים עמד על 0.67, 1.16, 0.43 ו-3.16 מקרים ל-100 שנות-אדם, בהתאמה.
החוקרים מסכמים וכותבים כי ממצאי המחקר מאשרים את הבטיחות והיעילות של פרדקסה לאורך שנתיים של מעקב, בהתאם לתוצאות מחקרים קליניים קודמים וכן נתוני עולם-אמיתי.

Therapeutic indications:
Pradaxa 75 mg and 110 mg: Primary prevention of venous thromboembolic events in adult patients who have undergone elective total hip replacement surgery or total knee replacement surgery.
* Pradaxa 110 mg and 150 mg
- Prevention of stroke and systemic embolism in adult patients with non-valvular atrial fibrillation (NVAF)
- Treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE), and prevention of recurrent DVT and PE in adults
BLACK BOX WARNING: DISCONTINUING PRADAXA IN PATIENTS WITHOUT ADEQUATE CONTINUOUS ANTICOAGULATION INCREASES RISK OF STROKE. Discontinuing Pradaxa places patients at an increased risk of thrombotic events. If anticoagulation with Pradaxa must be discontinued for a reason other than pathological bleeding, consider coverage with another anticoagulant [see Special warnings and precautions for use]. |
Contraindications:
- Hypersensitivity to the active substance or to any of the excipients listed in section 6.1
- Patients with severe renal impairment (CrCL < 30 mL/min) (see section 4.2)
- Active clinically significant bleeding
- Lesion or condition, if considered a significant risk factor for major bleeding. This may include current or recent gastrointestinal ulceration, presence of malignant neoplasms at high risk of bleeding, recent brain or spinal injury, recent brain, spinal or ophthalmic surgery, recent intracranial haemorrhage, known or suspected oesophageal varices, arteriovenous malformations, vascular aneurysms or major intraspinal or intracerebral vascular abnormalities
- Concomitant treatment with any other anticoagulants e.g. unfractionated heparin (UFH), low molecular weight heparins (enoxaparin, dalteparin etc), heparin derivatives (fondaparinux etc), oral anticoagulants (warfarin, rivaroxaban, apixaban etc) except under specific circumstances of switching anticoagulant therapy (see section 4.2) or when UFH is given at doses necessary to maintain an open central venous or arterial catheter (see section 4.5)
- Hepatic impairment or liver disease expected to have any impact on survival
- Concomitant treatment with systemic ketoconazole, cyclosporine, itraconazole and dronedarone (see section 4.5)
- Prosthetic heart valves requiring anticoagulant treatment (see section 5.1).
