NEW YORK (Reuters Health) – When decisions are being made about embryo transfer involving use of donor eggs, the age of the donor and embryo quality should be considered in order to reduce the risk of multiple births.
As in the December 1st issue of the American Journal of Epidemiology, Dr. Meredith A. Reynolds and colleagues from the Centers for Disease Control and Prevention, Atlanta, Georgia, reviewed data from 6936 in vitro fertilization (IVF) procedures performed on women ages 35 to 54 years who used donor eggs.
The authors note that 3320 (47.9%) of the procedures resulted in pregnancy and 2761 (39.8%) resulted in a livebirth delivery. Of the livebirth deliveries, 1185 (42.9%) were multiple births. The risk of multiple-birth delivery was not decreased by greater maternal age, they say, as is the case when women use their own eggs for IVF.
"Rates of multiple birth were related to number of embryos transferred and whether extra embryos had been cryopreserved, and they were high compared with those of IVF patients the same age who had used their own eggs," the investigators explain.
Transferring more than two embryos increased the risk of multiple birth among women who had extra embryos cryopreserved (an indicator of embryo quality), according to the authors. However, there was no corresponding increase in the chance for a livebirth.
"The findings of this study support current guidelines from the American Society for Reproductive Medicine and the Society for Assisted Reproductive Technology for making embryo transfer decisions," Dr. Reynolds and colleagues report.
"Further research should examine success rates for electively transferring a single, high-quality donor embryo in order to assess whether embryo transfers may be limited to one for certain recipients without significantly jeopardizing their chance of a livebirth," they explain.
