Laparoscopic radical nephrectomy is as effective as traditional open surgery during long-term follow up.
Laparoscopic radical nephrectomy is associated with less morbidity than open procedures. To assess its long-term effectiveness, surgeons from Washington University School of Medicine, St Louis, and centres in the United States, Canada and Japan, compared outcomes in 64 patients who underwent laparoscopic surgery with 69 subjects treated using open radical nephrectomy.
The average age of the subjects treated with laparoscopic and open radical nephrectomies were similar: 60.6 and 61.3 respectively. Neither average specimen weights (425 and 495 g respectively) nor Fuhrman grade (1.88 and 1.78 respectively) showed a statistically significant difference between the laparoscopic and open radical nephrectomy groups. Patients managed using open procedures showed significantly larger lesions on preoperative imaging than those treated with laparoscopic radical nephrectomy: 6.2 and 4.3 cm respectively.
Surgeons followed patients treated with laparoscopic radical nephrectomy for between zero and 94 months, median 54 months. In the open radical nephrectomy group, follow up lasted for between eight and 114 months, median 69 months.
