חולים ביפולרים לעתים קרובות סובלים מהפרעות חרדה ושילוב זה משפיע על התגובה לטיפול במיצבי מצב הרוח באנשים אלו.

מחקרים קודמים מצביעים על שכיחות גבוהה של הפרעות חרדה ודיכאון, אך יחסי הגומלין בין מצבים אלו אינם ברורים די צרכם. כדי לברר את יחסי הגומלין בין מחלה ביפולרית והפרעות חרדה, קבוצת חוקרים צרפתיים בראשותו של דר’ Chantal Henry מהשירות הפסיכיאטרי של Charles Perrens Hospital, Bordeaux, בדקה 318 חולים ביפולרים מאושפזים בין הגילאים 89-24.

באופן כללי, שבעים וחמישה חולים ביפולרים (24% של המדגם) סבלו מלפחות הפרעת חרדה אחת, כאשר 47% מהחולים האלו סבלו ממספר הפרעות חרדה. הפרעות החרדה השכיחות ביותר היו פוביות, הפרעות פאניקה (16% ו-11%, בהתאם) והפרעות כפייתית-טורדנית (3%).

מבחינה קלינית, נוכחותן של הפרעות החרדה לא השפיעה על חומרת המחלה הביפולרית, מאפיינים פסיכוטיים, שימוש בסמים ובאלכוהול או מספר ניסיונות התאבדות. בנוסף התברר שחולים ביפולרים עם הפרעות חרדה מגיבים פחות לטיפול במיצבי מצב הרוח. ממצאי המחקר העידו כי הטמפרמנט הדיכאני העלה את הסיכון להופעת הפרעות חרדה אצל חולים ביפולרים. להפתעת החוקרים, לא נמצא קשר בין תורשה, מחלות אפקטיביות במשפחות והופעת הפרעות חרדה אצל חולים ביפולרים.
