Colposcopy with histologic assessment should be the standard of care for women with persistent grade I or II cervical intraepithelial neoplasia (CIN) and cancer.
Conservative management of their care should be discouraged, European researchers say.
They found a high rate (31.2 percent) of underlying prevalent grade III CIN in a study of women with repeated Papanicolaou tests classified as grade I or II CIN and who tested positive for human papillomavirus (HPV).
German and British investigators comment: "The high rate of underlying prevalent grade III CIN and cancer found in our study indicates that conservative management of women with persistent grade I or II CIN should be discouraged.
"Colposcopy with histologic assessment should be recommended as the standard of care.
"However, for women with histologically proven grade I or II CIN, subsequent conservative management was safe in our study for those who were negative for HPV by type-specific polymerase chain reaction (PCR)."
