Any residual renal function decreases mortality risk in hemodialysis patients

WESTPORT, CT (Reuters Health) – Even very low levels of residual renal function in patients who are undergoing hemodialysis are associated with improved mortality compared with total lack of residual renal function.

Dr. Douglas Shemin of Rhode Island Hospital in Providence and associates prospectively followed 114 hemodialysis patients. At baseline, 58 patients had measurable urinary volumes and renal urea and creatinine clearances. During 24 months of follow-up, 50 patients died, the physicians report in the American Journal of Kidney Diseases for July.

The presence versus the absence of residual renal function was associated with an odds risk of 0.49 for risk of death (p = 0.013). The lower risk was independent of age, presence of diabetes, duration of dialysis or nutritional status. The presence of renal function appeared to be more powerful in predicting mortality risk than dialysis solute clearance, the investigators report.

Based on these findings, Dr. Shemin's group suggests that the "the preservation of residual renal function in hemodialysis patients may be just as important as improving solute clearance, anemia, and malnutrition in decreasing mortality."
