Orbital radiotherapy alone appears to be ineffective in treating restrictive thyroid myopathy and improving binocular function, say British researchers. A retrospective case note review of patients undergoing orbital radiotherapy for thyroid eye disease between 1992-1998 identified 79 case records. Fields of binocular single vision were analyzed prior to treatment and at three, 12, and 24 months after therapy, as an indicator of improvement in functional ability. Of the 79, a total of 27 patients had diplopia in primary position or a significantly reduced binocular field before undergoing radiotherapy; twelve had double vision in primary position prior to treatment, and 15 had a central but reduced binocular field. None of the 12 patients with double vision in primary position regained a central binocular field with radiotherapy alone. Of the 15 patients with a central but reduced binocular field, eight remained unchanged with treatment. In four patients there was an improvement in the field, while in three the field deteriorated.
