Salmeterol, a long-acting inhaled beta-agonist, may help prevent high-altitude pulmonary edema, according to the results of a double-blind, randomized trial reported in the May 23 issue of the New England Journal of Medicine.
This study also supports the concept that sodium-dependent clearance may be defective in those susceptible to high-altitude sickness. “Pulmonary edema results from a persistent imbalance between forces that drive water into the air space and the physiologic mechanisms that remove it,” write Claudio Sartori, MD, and colleagues from Centre Hospitalier Universitaire Vaudois in Lausanne, Switzerland.
“Beta-adrenergic agonists up-regulate the clearance of alveolar fluid and attenuate pulmonary edema in animal models.”
