Sildenafil citrate (Viagra) is not as reliable as intracavernosal injection of papaverine when interpreting penile vascular status accurately with colour Doppler ultrasound.
Sildenafil might be tried, however, as a first step in functional evaluation with the ultrasound in cases where psychogenic erectile dysfunction is suspected.
This might prevent the risk of prolonged erection with intracavernosal injection of vasoactive agents.
Investigators, from the Akdeniz University Faculty of Medicine, Antalya, Turkey, commented that the ideal diagnosis and therapeutic agent for erectile dysfunction would be an oral drug taken before colour Doppler ultrasound examination and sexual intercourse.
They examined whether the efficacy of oral sildenafil was optimal in the diagnosis of underlying pathology of erectile dysfunction in 41 patients. All patients underwent colour Doppler ultrasound examinations after the combined intracavernosal injection of papaverine 60 mg. and sexual stimulation. They were also examined after taking oral sildenafil citrate, 50 mg, plus sexual stimulation.
