Survival for patients with stage T3 low rectal cancers resected by total mesorectal excision (TME) is improved with preoperative radiotherapy, according to a report published in August issue of the Annals of Surgery.
Previous studies have yielded conflicting results regarding the benefits of preoperative radiotherapy and many surgeons have concluded that such therapy is unnecessary and only leads to greater morbidity and mortality.
However, no prospective data have been available to support this opinion. Dr. Conor P. Delaney and colleagues, from the Cleveland Clinic Foundation, assessed the outcomes of 259 patients with rectal cancer who were enrolled in a prospective database at their institution since 1980. All of the patients had undergone a curative resection with TME for a stage T3 tumor within 8 cm of the anal verge. The group included 92 patients who received preoperative radiotherapy and 167 who did not.
