The field of lung transplantation has experienced major advances over the last 10 years, but major obstacles continue standing in the way of achieving outcomes comparable to other types of solid organ transplantation.
Lung transplant recipients face a very high risk of morbidity and mortality related to the operation and posttransplant complications. The time period of risk may be divided into early and late by arbitrarily separating the times at 180 days posttransplantation.
The problems in the first 6 months deal with the graft, the operation, infection, and then rejection. Later events are dominated by the development of bronchiolitis obliterans syndrome (BOS), which is the leading cause of death 1 year or more after lung transplantation. While each of these issues may have an impact on long-term outcome, it is not clear that we achieve better outcome by reacting to events. Rather, it is probably better if prevention of events is the goal. To that end, this review will focus on donor and recipient sues that may affect outcome and specifically aim to reduce the incidence of BOS and its leading associated cause, acute rejection.
